My child,

has permission to get on a two-step ladder to paint

has permission to go on a single level scaffold

I am aware that during classes and events that precaution is taken to ensure that my child is safe and cared for. I understand that Studio B Artworks LLC has adult supervision by Mrs. Barbie Rodriguez (owner) and the studio's employed assistants and teachers. I fully understand that my child's participation in art events could possibly result in an unforeseen accident or injury, however unlikely.

I agree to waive any claim of any kind whatsoever, whether resulting from an injury or otherwise, and further agree to release, indemnify, and hold harmless Studio B Artworks LLC and its owner, employees, volunteers and/or representatives from any and all liability occurring as a result of my child's participation in art activities performed in the Studio.
